Surgical Closure of Patent Ductus Arteriosus in Preterm Infants: Clip Application or Suture Ligation

Abstract

Background: The aim of the study is to report single-center short-term results of preterm patients who underwent patent ductus arteriosus ligation via clip application and suture ligation through left anterior mini-thoracotomy.

Methods: Data of 22 preterm infants operated by the same pediatric cardiac surgeon who underwent PDA closure with left anterior mini thoracotomy technique between November 2020 and November 2021 at a single instution were reviewed.

Results: Twenty-two patients with a mean (±SD) gestational age of 25.9 (±2.1) weeks and a mean birth weight of 1039 (±452) g were operated using left anterior mini-thoracotomy technique. The lowest body weight was 600 g. A clip was placed on the PDA in 11 patients and PDA was ligated with a 2‐0 silk suture in 11 patients. There was no blood loss during the cases, no surgery related mortality or no significant difference in operation time between the cases.There were 8 mortalities after the operation (36%). There were 7 (31,8%) deaths in the postoperative first 30 days and 1 (4%) death between the postoperative first month and first year. The cause of the deaths were sepsis, NEC, hydrops fetalis, hepatoblastoma and intracranial bleeding. There was no statistically significant difference in the rates of morbidity and complication between the groups.

Conclusions: It was observed that there was no significant difference between left anterior mini-thoracotomy clip application and suture ligation approaches for PDA closure in premature infants in terms of blood loss, operation time, morbidity, complications and surgery-related mortality.

Prematüre Bebeklerde Patent Duktus Arteriyozusun Cerrahi Kapatılması: Klip Uygulaması veya Sütür Ligasyonu

Öz

Amaç: Çalışmanın amacı, sol anterior mini torakotomi tekniği ile ile patent duktus arteriyozuslarına (PDA) klip aplikasyonu veya sütürligasyonu yapılan prematüre hastaların tek merkezli kısa dönem sonuçlarını bildirmektir.

Yöntemler: Kasım 2020-Kasım 2021 tarihleri arasında aynı pediatrik kardiyovasküler cerrah tarafından opere edilen ve tek merkezde sol anterior mini torakotomi tekniği ile PDA’ları kapatılan 22 prematüre bebeğin verileri gözden geçirildi.

Bulgular: Ortalama (±SD) gestasyonel yaşı 25.9 (±2.1) hafta ve ortalama doğum ağırlığı 1039 (±452) gr olan 22 hasta sol anterior mini torakotomi tekniği ile opere edildi. En düşük vücut ağırlığı 600 g idi. 11 hastada PDA'ya klip yerleştirildi ve 11 hastada PDA 2-0 ipek sütür ile bağlandı. Olgular arasında kan kaybı, cerrahiye bağlı mortalite veya operasyon süresi açısından anlamlı fark yoktu. Ameliyat sonrası 8 ölüm (%36) oldu. Postoperatif ilk 30 günde 7 (%31,8) ölüm, postoperatif birinci ay ile birinci yıl arasında 1 (%4) ölüm vardı. Ölümlerin nedeni sepsis, NEC, hidropsfetalis, hepatoblastom ve kafa içi kanama idi. Gruplar arasında morbidite ve komplikasyon oranlarında istatistiksel olarak anlamlı fark yoktu.

Sonuç: Prematüre bebeklerde PDA kapatılması için sol anterior mini-torakotomi ile klip uygulaması ve sütürligasyonu yaklaşımları arasında kan kaybı, operasyon süresi, morbidite, komplikasyon ve cerrahiye bağlı mortalite açısından anlamlı bir fark olmadığı görüldü.
